Will your clinic accept my insurance?
Our clinic operates on a cash-based or out-of-network basis, and we do not have contracts with insurance providers.
We do, however, participate in the Community Care Network program for TriWest. Please ensure you have a referral before scheduling an appointment with us.
We accept a variety of payment methods.
If you're interested, we can provide you with a superbill.
What is a superbill?
A superbill is a detailed, itemized invoice provided by out-of-network health care providers to their patients for submission to insurance companies for reimbursement.
The process includes you (the patient/client) paying upfront (the day of the visit), requesting a superbill, and then submitting it to your insurance company.
How do I obtain a superbill from you?
Please note that we do not automatically generate a superbill with the additional codes included.
Typically after each visit, you will find a standard receipt in our electronic medical system, PTEverywhere.
If you require a superbill, please inform your therapist at the beginning of your care, and they will ensure that the necessary codes are included.
You can print the superbill (or the standard receipt) directly from PTEverywhere.
How do I know if I can submit a superbill to my insurance provider, and how do I submit it?
 Before visiting us, please check with your insurance company about submitting a superbill. |  Reimbursement is not guaranteed and will depend on your out-of-network benefits. |
